The JNC 8 guidelines recommend that lifestyle modifications be the first line of treatment for all patients with hypertension. These modifications include weight loss, physical activity, a healthy diet, and limiting alcohol intake. For patients with stage 1 hypertension who do not have any other risk factors, the guidelines recommend starting with a single medication. For patients with stage 2 hypertension or those with other risk factors, such as diabetes or chronic kidney disease, the guidelines recommend starting with two medications.


The JNC 8 guidelines also provide recommendations for specific patient populations, such as older adults, African Americans, and patients with diabetes. For example, the guidelines recommend a higher blood pressure target of less than 150/90 mmHg for older adults and African Americans, while patients with diabetes should have a blood pressure target of less than 140/90 mmHg.
